I Bought Tirzepatide 2-3 Months Ago and Never Mixed It — Is It Still Good?
A vial sitting sealed and dry in a closet for 2-3 months is a very different situation than a punctured, reconstituted one — and the answer depends on which form of tirzepatide you actually have.
This is one of the most common questions we get, almost word for word: "I bought tirzepatide a couple of months ago, never got around to mixing it, it's just been sitting in my closet — is it still usable?"
The short answer: it depends on which form of tirzepatide you have, and the good news is that the lyophilized (freeze-dried), never-reconstituted form is the most forgiving state a peptide can sit in.
If it's a lyophilized powder vial (compounded / research peptide)
This is almost certainly what you have if the product needed to be "constituted" — brand-name pens arrive pre-mixed and don't require this step. Lyophilized tirzepatide is the most stable form the compound can be in, precisely because degradation reactions like oxidation and deamidation both need water to proceed, and freeze-drying removes it.1
Published stability data for lyophilized tirzepatide generally supports:
- Refrigerated (2-8°C): roughly 18-24 months
- Frozen (-20°C): roughly 24-36 months
- Room temperature (20-25°C): commonly cited in the range of 6-12 months, depending on humidity and light exposure
A closet at 2-3 months in is well inside that room-temperature range, provided two things hold true: the vial hasn't been exposed to genuine heat extremes (direct sun, near a heater, a hot car, a South African summer attic), and it still looks the way it should.
One caveat worth being upfront about: room-temperature stability data varies by compounding pharmacy and formulation — whether lyoprotectants like trehalose or mannitol were used during freeze-drying, for instance, measurably affects how well the powder holds up.2 If your vial came with a printed beyond-use date or specific storage guidance from the pharmacy or supplier, that guidance overrides any general range like the one above.
If it's a brand-name pen (Mounjaro or Zepbound)
This is a genuinely different situation, and worth flagging clearly: brand-name tirzepatide pens are not lyophilized — they're already in solution. Eli Lilly's own labeling allows unopened pens to sit at room temperature (up to 30°C/86°F) for only 21 days before they must be discarded, regardless of whether they've been used.3 Two to three months well exceeds that window. If what you have is an unopened pen that's been sitting out (not a powder vial), the manufacturer's guidance is that it should not be used.
The bottom line
- Powder vial, sitting sealed and dry, no heat extremes, still looks normal → very likely still fine to reconstitute; visually inspect first
- Brand-name pen, out of the fridge for 2-3 months → per manufacturer labeling, this is past its usable window and shouldn't be used
